The Lazkao Care Ecosystem: Towards the Transformation of Long-Term Care

Care ecosystems are based on the development of local initiatives and projects that enable more personalised, flexible care that is closely linked to the community, with the aim of ensuring full citizenship for everyone, including those who require long-term care. To this end, social innovation is being promoted, home care is being strengthened and the transformation of the care model is being encouraged, amongst other measures, through collaboration between social and health services. The ultimate aim is for everyone to be able to pursue their own life plan, regardless of the type of support they require or where they live.


In Lazkao, a process has been launched to carry out a joint assessment of the current care situation together with the local community. This assessment takes a community-based approach to care, enabling solid and coherent progress towards the development of the town’s own care ecosystem. The aim is therefore to bring together in a single forum all perspectives and experiences relating to long-term care, fostering a shared dialogue that will first enable us to understand the current situation and then, based on that analysis and reflection, jointly develop a common roadmap for the future.
